K-12 Physical Night

$10 Physicals for any student in grades K-12 who goes to school or lives within the SM West attendance area
 
Thursday, May 9 from 5:30pm-7:30pm at SM West High School

NOTE:  Physical forms MUST be signed by parent/legal guardian PRIOR to the physical.  Please either sign the form ahead of time or attend with your child.

Walk in patients will be seen on an as-available basis.

Families who qualify may apply for a West is Best Scholarship to assist with the cost of the physical.  DO NOT mail your application to West is Best.  Bring it with you the evening of the event.
